Primary Care

All Medicare Part D medications are fully covered, including specialty drugs — no copays or gaps in coverage for PACE members.
Dentistry & Vision
Routine dental care, eye exams, prescription eyeglasses, and hearing aids — services traditional Medicare often doesn't fully cover.
Transportation
Door-to-door transport to PACE day centers, medical appointments, and community activities — safe, reliable, and fully covered.
Therapy Services
Physical, occupational, and speech therapy to help maintain strength, mobility, and communication — on-site at the PACE day center.

Who Qualifies

Age 55 or older

PACE serves adults who are 55 years of age or older and need a nursing home level of care.

Live in a PACE service area

You must reside within the geographic service area of a PACE organization in your state.

Medicare or Medicaid eligible

You are eligible for Medicare, Medicaid, or both. PACE covers costs fully for dual-eligible members.

Able to live safely at home

At the time of enrollment, you must be able to live safely in the community with PACE support.

The "Dual-Eligible" Advantage

If you have both Medicare and Medicaid, PACE is usually provided at $0 cost to you. We handle all the paperwork and coordination.

Find the answers you need

For participants who qualify for both Medicare and Medicaid, PACE is provided at no cost — no copays, no deductibles. If you only have Medicare, there may be a monthly premium for non-Medicare-covered services. Our team will walk you through your specific coverage during enrollment.
PACE participants receive their primary care through the PACE interdisciplinary team, so you would transition to PACE physicians and specialists. Many participants find the PACE care team — which meets regularly to discuss each member — provides more coordinated care than they received before.
No — attendance at the PACE day center is based on your individual care plan. Some members attend several days per week; others may receive care at home more often. Your care team will work with you to determine the right schedule for your needs and preferences.
PACE coordinates and covers all necessary hospitalizations and nursing facility stays when needed. The PACE team works closely with hospital staff to ensure smooth transitions, and the goal is always to return you home as safely and quickly as possible.
Yes. PACE enrollment is voluntary and you can disenroll at any time. If you decide to leave, your coverage will revert to original Medicare and Medicaid, and our team will assist with a smooth transition of your care records and prescriptions.
