What is PACE?
PACE — Program of All-inclusive Care for the Elderly — is a unique model that provides comprehensive medical and social services to frail seniors, coordinated through an interdisciplinary team, so they can remain living at home and in their communities.
Every service — from doctor visits to transportation — is organized and covered under one integrated plan.
Doctors, nurses, social workers, therapists, and nutritionists collaborate daily around each participant's individual needs.
PACE is designed to help seniors stay in the home they love — avoiding unnecessary nursing home placement.
Our Services
PACE covers over 55 services — at no additional cost to eligible participants.
Who Qualifies
PACE is a lifeline for seniors who qualify — covering all healthcare costs for those who meet these criteria.
PACE serves adults who are 55 years of age or older and need a nursing home level of care.
You must reside within the geographic service area of a PACE organization in your state.
You are eligible for Medicare, Medicaid, or both. PACE covers costs fully for dual-eligible members.
At the time of enrollment, you must be able to live safely in the community with PACE support.
If you have both Medicare and Medicaid, PACE is usually provided at $0 cost to you. We handle all the paperwork and coordination.
